I, the undersigned, hereby declare that I voluntarily choose to participate in the above-mentioned event/activity (Trip to Yas Water Park(Ladies only)).

I fully release, waive, and discharge the University of Dubai, its management, employees, and representatives from any liability, claims, demands, or actions arising out of or related to any loss, damage, or injury — including death — that may occur to me or to any property belonging to me while participating in this activity, which I acknowledge involves certain inherent risks.

During this event, I agree to abide by all University regulations and policies, including those outlined in the Student Code of Conduct. I understand that the consumption of alcohol is strictly prohibited and that any violation of University rules and regulations may subject me to disciplinary action.

By signing this document, I confirm that I have carefully read this Liability Waiver / Student Event Declaration and Hold Harmless Agreement, that I fully understand its contents, and that I am signing it voluntarily and of my own free will.
